b) School Health Service Booklet on "Health Check and Immunisation for P5 Pupils"
c) 'Can you hear me?' Booklet
There are contacts given in the letter. Alternatively, you may directly access HPB website at https://childconsent.hpb.gov.sg with your SingPass to provide consent for immunisation and to provide medical information of your child online.
